What is Nursing Informatics?

Nursing Informatics is the integration of clinical nursing with information management and computer processes. It is a relatively new focus in health care that combines nursing skills with information technology expertise. Nurse informatics specialists manage and communicate nursing data and information to improve decision making by consumers, patients, nurses and other health care providers.

The nursing process has four main steps: planning, implementation, evaluation, and assessment. However, because information management is integrated into the nursing process and practice, some nursing communities identify a fifth step in the nursing process: documentation. Documentation and patient-centered care are the core components of the nursing process. Automated documentation is vitally important, not just for nursing, but for all patient care. Up-to-date, accurate information at each step of the nursing process is the key to safe, high quality patient-centered care.

The successful implementation of information systems in nursing and health care requires the following: First, it is necessary to have well designed systems that support the nursing process within the culture of an organization. The second requirement is having the acceptance and integration of information systems into the regular workflow of the nursing process and patient care. Finally, it is important to have resources that can support the previously mentioned factors. One of the most effective and valuable resources a healthcare organization can add is a Nurse informatics specialists.

Nursing Informatics Specialists

Nursing Informatics Specialists are expert clinicians with an extensive clinical practice background. These individuals have experience in utilizing and implementing the nursing process. These nurses have excellent analytical and critical thinking skills. They also understand the patient care delivery workflow and integration points for automated documentation. Having additional education and experience with information systems is also important for this occupation. Finally, Nursing Informaticists are excellent project managers because of the similarity between the project management process and the nursing process.

To be competitive in this field one should become familiar with relational databases by taking a class about database structure. They should also become competent and comfortable with MS Office, especially Excel, Access and Visio.

Why these jobs are Important to Healthcare?

Nurse and health informatics bring a great deal of value to patients and the health care system. Some examples of how they provide value include:

  • Increasing the accuracy and completeness of nursing documentation
  • Improving the nurse's workflow
  • Automating the collection and reuse of nursing data

HIMSS and RHIO

To provide some background on the field of healthcare / nursing informatics, there are some governing bodies for this field. The Healthcare Information and Management Systems Society (HIMSS) is the main governing body for health care and nursing informatics professionals. This group, formed in 2004, has the following four goals: NI awareness, education, resources (including websites), and RHIO (Regional Health Information Organization).

RHIOs are also known as Community Health Information Networks (CHINs). These are the networks that connect physicians, hospitals, laboratories, radiology centers and insurance companies.They all share and transmit patient information electronically through a secure system. Those organizations that are a part of RHIOs have a business interest in improving the quality of healthcare being administered.

Steps to a Job in This Field

To enter into the nursing informatics field, typically you need a minimum of a four year degree. There are specific health informatics degrees available. Earning your Bachelor's of Science in Nursing (BSN) is also a requirement before sitting for the ANCC certifications test for Nursing Informatics. Some individuals start with just a two year degree or diploma, but continue on to earn their BSN before becoming certified. Although there are several different routes for getting into the field, the most favored manner is to earn a Master's in Nursing Informatics from the start, however, most individuals start their career prior to earning their master's degree.

Most nurses who are in the informatics field start in a specialty area, such as the Intensive Care Unit (ICU), Perioperative Services (OR), Med-Surg, Orthopedic Nursing, or Oncology, just to name a few, and work in that specialty field for an extended period. Working in a specialty area helps nurses get to know the normal working processes and routines as well as understand the patient care process in their specialty. They usually are experts at their specialty and then develop interests in computerized documentation or some other technological healthcare focus. They then tend to gradually move into an information systems clinical support role.

If you have an interest in nursing and technology, this might be a career that can match both of these skills into one rewarding job.

Veins Explained

To understand how varicose veins develop, you need to know a little bit about how blood moves through the body. Essentially, there are two types of major circulatory vessels in the body: the arteries, which channel blood from the heart to the extremities, and the veins, which bring blood from the extremities back to the heart. Of all the veins in your body, leg veins work the hardest to carry blood back to the heart. To accomplish this difficult task, your legs are equipped with specially designed one-way valves that keep the blood moving in the right direction. As efficiently as these valves work, your legs still fight a constant battle against the natural pull of gravity. If the veins can not move the blood efficiently or the one-way valves that prevent the blood from backwashing fail to close properly, blood collects in the legs. This pooling of blood then stretches the vein, causing swelling and injury to the vein's walls (3).

Depending on the extent of the enlargement, these veins can show up as either spider or varicose veins. Spider veins are broken capillaries and small veins that appear as jagged red, or purple lines on the surface of the skin. While they are not painful, they can be extremely unattractive. Varicose veins, on the other hand, are thick veins that run deeper beneath the skin. They are far larger and much less attractive than spider veins – and they can make your legs feel fatigued, heavy, achy or even itchy. Varicose veins can also cause burning, throbbing, cramping and restlessness.

While these conditions are not usually dangerous, severe cases can lead to chronic venous insufficiency – a persistent inability of the leg veins to adequately return blood back to the heart (4). Varicose veins are also associated with the development of skin ulcers or a chronic inflammation of a vein – a condition known as phlebitis. Phlebitis is often accompanied by formation of a blood clot, a dangerous situation since the clot can move from the leg vein and travel to the lungs (2).

Get a Leg Up with Diosmin

Standing or sitting all day makes it harder for blood to move up from the legs and back to the heart. In one clinical trial, people who suffered from pain and swelling due to varicose veins experienced significant improvement in their symptoms after taking Diosmin for a few months (6). A review of studies also found that, among more than 5,000 patients with vein disorders, the vast majority not only saw a reduction in their discomfort, they actually saw the swelling in their calves and ankles disappear (7). Across the board, research shows that a daily dose of 500 to 1,000 mg of Diosmin improves the health of leg veins and brings tremendous relief from pain, night cramps and leg heaviness (8,9,10,11). In two studies pitting Diosmin against other flavonoids like rutin, grapeseed extract and red wine, Spanish investigators found that Diosmin was considerably more effective at reducing the number of metastatic melanoma cells (a potentially deadly type of skin cancer cells that spread through the body) than any of the other flavonoids tested (19,20). Other studies have found that Diosmin puts the brakes on the spread of cancer cells in the mouth (25).

Although it could be years before science conclusively proves Diosmin's cancer benefit, this nutrient can not be beat for supporting vein health. Along with taking supplemental Diosmin, there are several simple strategies you can employ to encourage healthy circulation. Eating a high-fiber, anti-oxidant rich diet and getting plenty of exercise can help keep veins in top form. And remember to take a break and put your feet up every now and again.

Does Conjugated linoleic acid (CLA) have any bad side effects? Does it work for weight loss? This is due to the release of a meta-analysis (basically a review of several scientific studies) published in the May 2007 issue of the American Journal of Clinical Nutrition which concluded that 3.2g / d of CLA can induce moderate weight loss in humans. CLA is an attractive weight loss supplement and many people use conjugated linoleic acid as part of a fat loss supplement stack (ie a group of compounds and herbs taken together to maximize effects) because unlike many other weight loss supplements it is not a stimulant and you do not suffer the nasty side effects of getting the jitters, increased heart rate, or worse – increased blood pressure. This is especially as there are not many effective options for non-stimulant fat burners on the market.

Let's now look at two more studies that involve people taking supplemental CLA. The first study was again published in the American Journal of Clinical Nutrition. The researchers found that when people supplemented with 3.2 g / d of CLA (this is generally the recommended dosage for weight loss purposes) they burned more fat and more specifically they burned more fat when they slept!

This is true.

Not only did the subjects that took CLA burned more fat when they slept, the fat they burned was NOT fat they had recently eaten; it was actually stored body fat that they were burning. This study gets even better as the researchers reported that the individuals that took CLA had decreased urinary protein losses. In other words the CLA group had improved protein retention when they slept. These are really interesting findings. If I owned a supplement company that sold large dosages of CLA – my new headline would be "CLA – Scientifically Proven to Burn More Body Fat and Build Muscle while you SLEEP."

So CLA can work for weight loss but are there any side effects? Again in 2007, another study was published that looked at the effects of CLA on weight loss, this time in obese people. In this study, the participants were given CLA dosages of 0, 3.2, or 6.4 grams / day. At the end of the study the group that received the 6.4 g / d of CLA experienced a significant increase in a compound called C – reactive protein or CRP for short. C – reactive protein is a protein that is released from your liver. It is commonly used in the medical field as a general marker of the level of inflammation in your body – higher CRP means more inflammation.

While there was an increase in CRP, it was truly not clinically significant as CRP levels remained below what is considered normal (Normal CRP levels are 3mg / dL). It is also important to see realize that the people in the study that had increased CRP as a result of taking a CLA supplement were taking 2x the 'recommended' dose for weight loss and also that people that obese normally have higher CRP levels (this might have come into play here as well). The group that only took 3.2 grams per day did not have any increase in their CRP levels.
Based on the findings in the studies that I've mentioned above and the review of studies from article in the American Journal of Clinical Nutrition is seems that 3.2 g / d of CLA can be safely taken to boost weight loss.

The next question that you should ask is …

What do you do with these findings? At the moment not a whole lot … CLA is a nice add-on to a fat loss program but "add-on" is the key word. CLA is perfect for nutrient stacking as combing these effects with a supplement like green tea extract (which has also been shown to boost weight loss), increased non-exercise physical activity, and multiple meals throughout the day will surely boost your fat loss. Pregnancy In The Stone Age – Can We Learn Something?

The woman who became pregnant during the Stone Age faced huge risks compared with today's mother-to-be. There was no way to control bleeding or infection; Caesarean section was not an option. That we survived as a species seems remarkable – until you dig a little deeper.

The outcome of pregnancy depends on the underlying health of the mother, nutrition before and during pregnancy and the threat of infection. In all those areas the woman of 50,000 years ago was better off than her counterpart today. How is that possible in an age when there was no plumbing, no medical care and no protection from infectious diseases?

Better diet, better pregnancy

The primitive woman's diet was less likely to be deficient in important nutrients than that of today's young girls. (Ref 1) A study from the University of Cincinnati Medical Center has confirmed earlier reports that adolescent and adult pregnant women take in too little iron, zinc, folate and vitamin E.

A woman who begins pregnancy without sufficient calcium, vitamin D and other bone-building nutrients increases her risk of developing osteoporosis in middle age. But that's not the end of the story. Her infant may also be at greater risk of fracture in the future. Osteoporosis of middle age is at least partly programmed before birth, especially if the mother smokes and has little physical activity. (Ref 2, 3, 4)

Most laypersons believe that Stone Agers were hearty meat-eaters. Anthropologists know that is not so. They lived on a predominantly meat diet for only about 100,000 years, from the time that Homo sapiens developed keen hunting skills until the advent of farming. Before that time meat came from carrion and small game. The bulk of their calories came from vegetables, fruit, roots and nuts.

Plant foods contain everything that a pregnant woman needs, including vitamins, antioxidants, protein and minerals. Modern vegetarians often become deficient in vitamin B12 but small game, birds' eggs and the deliberate or accidental inclusion of insects in the Stone Age diet provided more than enough of that critical nutrient.

Folic acid deficiency in early pregnancy leads to defective formation of the infant's brain and spinal cord. Those abnormalities are much less likely among the babies of mothers who receive an adequate amount of folate, at least 400 micrograms per day. So few women eat enough green leafy vegetables to boost their folate levels, the US government requires that manufacturers of baked goods add it to their products.

Obstetricians have been prescribing multivitamins for their pregnant patients for decades but it is only in recent years that studies confirmed the wisdom of that practice. In 2002 the American Medical Association reversed a position of long standing and recommended that everyone, with no exceptions, needs a multivitamin / multimineral preparation every day in order to avoid subtle but health-damaging inadequacies of these nutrients. Taking a multivitamin reduces the risk of congenital defects of the newborn, especially those that involve the heart. Preeclampsia is a serious, sometimes fatal complication of pregnancy. Women whose intake of vitamins C and E is low have a threefold greater risk of that condition. (Ref. 5, 6)

Would these mostly vegetarian early Stone Agers have become iron-deficient? Not likely. Their diet was rich in iron as well as in Vitamin C that facilitates iron absorption. Under those conditions iron deficiency would have been rare. Cereal grains interfere with iron absorption, which explains why iron-deficiency is common in societies that subsist primarily on grains. However, one of the main reasons why Stone Age women were unlikely to be iron deficient is that they did not have nearly as many menstrual cycles as modern women do.

In a primitive society the onset of menses is about 5 years later than that of American young women. Modern hunter-gatherers, like the oldest Stone Agers, are either pregnant or nursing during most of their childbearing years and they only menstruate a few times between weaning one child and conceiving another. In those groups breastfeeding does suppress ovulation because it is literally on demand, ie, every few minutes, even throughout the night. For a modern breastfeeding mother, on demand often means no more frequently than every couple of hours and perhaps once or twice a night after the third or fourth month. Thus menses return in spite of nursing and monthly blood loss continues.

The fish-brain connection

Beginning about 150,000 years ago our ancestors discovered seafood. The increased intake of fatty acids in fish and shellfish initiated the great advance in brain size and complexity that allowed humans to progress more quickly in the next 100,000 years than they had in the preceding million. Enormous gains in toolmaking and the development of language and group communication followed.

The human brain is composed mostly of water but the solid portion is mostly fat. The body can not manufacture the omega-3 and omega-6 fats that make up so much of the structure of the brain and eye so we need them in our diet. Maternal deficiency of these nutrients, especially omega-3s, prevents the newborn brain and eyes from reaching their full potential. The best source of omega-3 fats is fish; nuts and leafy green vegetables are also good sources.

Omega-3 and omega-6 fatty acids are found in every cell of the body. They allow efficient flow of nutrients, regulate nerve impulses and keep inflammation in the right balance. In a proper diet there is an equal amount of omega-3 and omega-6 fats. That allows the immune system to fight infection, a real threat that humans faced from the Stone Age until the age of antibiotics, a mere 70 years ago.

The advantage to the baby of a diet that is rich in omega-3 fats is obvious but mothers need it, too. Nature protects the unborn infant by tapping into the mother's stores of omega-3 fats. A woman whose intake of omega-3 fatty acids is low during the months and years preceding pregnancy will develop a deficiency of her own. This becomes worse with succeeding pregnancies if her intake of omega-3s remains low. Postpartum depression affects about 10 percent of women following delivery and it is associated with a deficiency of omega-3 fats. (Ref 7, 8)

The newest epidemic

There is one complication of pregnancy that never occurred in the Stone Age: type 2 diabetes. No disease in modern times has risen so fast. It has increased several-fold since the 1950s; between 1990 and 2001 it rose by 61 percent. Gestational diabetics (Ref. 9) are those who do not yet have the full-blown disease but they can not process blood sugar (glucose) properly during pregnancy. About half of them will develop frank diabetes in the years following delivery of their infant.

Most of us know type 2 diabetes, which was once referred to as adult-onset diabetes, as the disease that our grandparents developed in their later years. It's no longer uncommon to find it in adolescents, even in grade-schoolers. As it has dipped into the younger generation it has alarmed – but not surprised – physicians to find that it is no longer a rarity in obstetric practice.

How can we be so certain that the pregnant Stone Ager did not have diabetes? This is a lifestyle disease that has three major associations: a low level of physical activity, a diet that is high in refined grains and sugars, and obesity. Those conditions simply did not occur during the Stone Age. Their lifestyle demanded strenuous effort. Grains of any sort were not part of their diet because they require tools and controlled heat. Sugar as we know it simply did not exist and honey was an occasional lucky find. Obesity would have been non-existent, as it is today among the planet's dwindling populations of hunter-gatherers.

Diabetic mothers have more complications of pregnancy than normal women do. Their babies are 5 times as likely to die and are 3 times as likely to be born with abnormalities of various organs.

They kept germs at bay

Common wisdom states that Stone Age people were an infection-ridden lot but that simply is not true. They had powerful immune systems because of high levels of physical activity and a remarkably varied diet. Between the protective antibodies that a mother passed across the placenta and those that she conferred on her newborn via breastmilk, Stone Age babies had more protection against the germs of the day than modern infants do.

Sexually transmitted diseases do not spread very far or very fast when people live in small isolated bands as they did during the Stone Age. The likelihood that today's pregnant female will have at least one of these infections is more than 50 percent (Ref. 10). The impact on babies can be severe; some die, some will be brain-damaged.

Choice and consequences

Tobacco, alcohol and illicit drugs have produced a generation of infants with problems that Stone Age babies never faced. Mothers who smoke have infants that are smaller than the norm and whose brain development may be compromised. Alcohol or cocaine use by the mother during pregnancy results in stunted growth, congenital defects and other severe problems.

Given a choice, none of us would want to live in a Stone Age world but we have neutralized the almost miraculous medical advances of the last century. We have allowed our daughters to be less physically active and to subsist on a marginal diet. If we could reverse those two factors alone there would be a dramatic decline in prematurity and other complications of pregnancy.
The lessons that we can learn from the Stone Age are not subtle, obscure or beyond our capacity to imitate them. We can produce the healthiest generation ever by making better choices for our children and for ourselves.

With recent popularity of eating steamboat in winter season, many people have steamboat more frequent as usual. However there is also some health conscious and healthy diet practicer and analyst complain on consuming steamboat is not healthy, potential in having high cholesterol, overheat, and even as worst as contributing cancerous element to our body. Actually there is no bad natural foods, only wrong eating methods. Wrong Perception 1: Mutton and Beef has too high fat quantity level, whereas fish balls and meat balls has lower fat. The selection of ingredients of steamboat will directly impact the fatty and heat level we consume. But particularly in steamboat, green vegetables with big leaves is subjected in easier to absorb more oil from the soup, mushrooms also has the same function, hence it makes the green healthy vegetables to a high fatty and oily vegetables, no longer healthy.

Suggestion: Instead of choosing big leave green vegetables, why not select those stem type vegetables like seaweed, lotus, carrot, radish, potato, and yam. Since these type of vegetables will not absorb so much oil element in the soup, more over the high fiber in these vegetables can help to remove the oil that park inside our stomach.

Wrong Perception 3: The calorie level between different sauces are the same. Most of the time we neglected the sauces also contributed high calories into our diet, as usually we will consume a lot of sauces for better taste and flavor. Example the frequent use sauce like garlic oil sauce, sesame sauce, chilies sauce and other sauces will have easily 100 calories in 1 big tablespoon.

Suggestion: Replace these above high calories sauces with fresh cut chillies, spring onion, white pepper into soy sauce, which will reduces at least half of the calories in the sauces. You can add in 2 teaspoon of vinegar to help in dissolving fatty oil in the foods.

Wrong Perception 4: Finishing up steamboat meal with desserts or main dish (noddles, rice, etc). Usually a lot of us will finish up the steamboat with some main dish like fried rice or noodles and some sweets. Actually this is not right as we might over consume with excessive calories.

Suggestion: The right way is to consume only 60-70 percent fullness, if you want to have desserts or main dishes. If you have taken too much starch base foods, just avoid main dishes and drink some teas to help in digestion.

Wrong Perception 5: Slowly enjoy eating steamboat for hours while chit chat along. Even we understood the benefit of eating slowly, but it does not apply in eating steamboat, since the steamboat soup base once it is cooked for more than 90 minutes, the nitrite level in the soup will increase and consuming more nitrite is not good.

Suggestion: Control your steamboat eating session around one and half hours, no longer than this duration.

Juice Plus is a supplement that has been in existence since 1993. I looked into the product at one time as a possible source of nutritional benefit for my family. I was even interested in becoming a distributor so that I could earn a residual income, since so many people are looking for ways to improve their nutrition just like you and I. I will not say outright that the Juice Plus is a scam. I have thoroughly looked into many business opportunities, especially health related companies, and I will say that there are many fraud companies out there. I think that Juice Plus is actually one of the better companies out there, but in my search for the best quality source of vitamins, minerals, antioxidants, and general nutritional supplements … a little bit of research let me know that Juice Plus + was not for me. Let me tell you why I decided that it was not for me.

The Juice Plus Company

Juice Plus is manufactured by a company called Natural Alternatives International in Santa Monica, California and distributed by National Safety Associates (NSA) in Tennessee. NSA was started in 1970 and before introducing Juice Plus, was known for other multilevel-marketed products. Juice Plus scam? No, they are just a smart company that seems to follow the business trends of the decades. Do you remember all of those water filter companies that were popular in the 1980s? Well this company sold home fire detectors when they were popular in the 70s, water filters in the early 80s, and air filters in the late 80s. They also sold educational games at one point.

While some companies in the past have made multilevel-marketing look bad, there is nothing wrong with using the method of sales distribution known as multilevel-marketing. If done right, it can benefit more people than traditional marketing, save people money and can also help get a product out to many who may have never learned about the product.

So from a business standpoint, it looks like they have the right idea. They follow the trends.

The Juice Plus Products

Their main products are the Juice Plus, which are not juice really, but powder in capsules. This is better than juice because juice supplements like the popular Acai berry juice, noni juice, Mona vie juice or Mangosteen juice products will begin to oxidate as soon as you open the bottle. This liquid oxidation is harmful to the body. The Juice Plus blend include great ingredients that come from fruits such as acerola berries, apples, cranberries, oranges, papaya, peaches, and pineapple. Their Juice Plus + contains vegetables that are sourced from barley, beets, broccoli, cabbage, carrots, kale, oats, parsley, spinach, and tomato. So to answer the question is Juice Plus fraud, I say that there is no Juice Plus scam, since they sell real products with decent ingredients.

It has become general knowledge that by increasing your fruit and vegetable intake on a daily basis, that you can improve your health and wellness. Eating more fruits and vegetables will actually help your body also get rid of free radical cells as well as a natural antioxidant in the body. I just do not feel that this product has enough fruits and vegetables in it for me is all.

A Few Juicy Problems

Their Juice Plus Gummies supplement for kids were found to have around 80% corn syrup and 10% beef gelatin, making the product almost without nutritional benefit. So these are not much better than the gummy vitamin / mineral bears that you can buy at your local store. Both are basically just candy!

I assume that their other products probably have a higher quality of nutrition than this but I saw some other problems that made me shy away.

The Biggest Problem

I could not find any of the product labels on their site.

The biggest problem that I saw was that in all of the poking around on their company website, I could not find any of the product labels for any of their products! What are they trying to hide? Sure they list a few fruits and vegetables, but they do not say how much of each fruit that they use. They do not show you the nutritional facts so you can see how much sugar and natural or artificial ingredients are in each product.

I did not like that I had to buy the product to see the product labels.

Some other problems that I saw …

1. The products were not unique enough

2. There was not enough research and science

3. The testimonials were lacking

4. The products were not manufactured to high enough standards to make me want to pay the extra money for them

5. Not all of their ingredients were organic and / or plant-based.

Juice Plus Research

One thing that caught my interest when first evaluating Juice Plus was that the product had at least been in scientific journals such as Asia Pacific Journal of Clinical Nutrition, Evidence-Based Complementary & Alternative Medicine, Nutrition Research, Journal of Human Nutrition and Dietetics and Integrative Medicine.

But wait a second! I soon discovered that of the the published peer-reviewed studies, only one was conducted independently! The others were all funded and / or authored by the manufacturer, the main distributor, or by individual Juice Plus distributors. That made me a little leery!

Juice Plus Testimonials

One serious mistake that was made, was in choosing former professional football player OJ Simpson to be celebrity endorser of the product. Simpson was given a multi-year six-figure contract to sign on with NSA as a icon for the company. I personally like to see a company that has a product that is so good that star athletes use the supplement on their own, simply because they like the added performance gain and benefits that they get while using them.

The testimonial of OJ Simpson was thrown even further out after he was charged with murdering his wife and her friend Ronald Goldman.
